I have an IUD. Can I still take a fertility test?

Fertility testing is as quick as a doctors appointment.

For instance, an AMH test involves a quick stop at the clinic for a blood test. It’s the same for hormones such as FSH, LH and so on.

Its an essential step to help us understand our reproductive health and how our baseline hormones function.

However, do the levels remain the same with an IUD?

This blog will explain how an IUD affects fertility tests, what you need to know, and how to proceed.

What is an IUD?

An IUD is a small, T-shaped device inserted into the uterus to prevent pregnancy.

There are two main types of IUDs: hormonal and copper, which is non hormonal.

Hormonal IUDs release progestin, while copper IUDs release copper ions, both creating an environment that is inhospitable to sperm.

Can you undergo fertility testing with an IUD?

Yes, you can undergo fertility testing with an IUD in place.

Most fertility tests focus on hormone levels, ovulation, and overall reproductive health, which are not significantly impacted by the presence of an IUD.

However, there are chances that your hormonal levels may be altered due to a hormonal IUD. Unless your hormonal levels are drastically different, they don’t usually request for a re-test or ask you to come off IUD for a few months.

Types of fertility tests and their relevance with an IUD

  1. Hormonal blood tests:

    • What they measure: Levels of hormones like FSH, LH, estradiol, progesterone, and AMH.

    • Relevance with IUD: The presence of an IUD ideally should not affect the results of these blood tests. However, there are chances of minor fluctuations at times, and you may be asked for a re test in the case of major fluctuations.

  2. Ovulation tracking:

    • What it involves: Monitoring ovulation through basal body temperature, ovulation predictor kits, or ultrasound.

    • Relevance with IUD: Hormonal IUDs may suppress ovulation, making it harder to track ovulation. Copper IUDs do not affect ovulation.

  3. Ultrasound:

    • What it shows: Ovarian follicles, uterine lining, and structural abnormalities.

    • Relevance with IUD: An IUD may be visible on an ultrasound but does not interfere with the assessment of ovarian and uterine structures.

  4. Hysterosalpingography (HSG):

    • What it involves: An X-ray procedure to check the fallopian tubes and uterine cavity.

    • Relevance with IUD: Typically performed after IUD removal to avoid complications.

  5. Semen analysis for male partners:

    • What it measures: Sperm count, motility, and morphology.

    • Relevance with IUD: Not affected by the presence of an IUD in the female partner.

Do's and Don'ts for Fertility Testing with an IUD

Here’s our checklist on what to follow, when testing for your fertility with an IUD.

Do's

  • Consult Your Doctor: Discuss your fertility goals and the impact of your IUD with your healthcare provider.

  • Schedule Tests Appropriately: Plan tests that may require IUD removal, like HSG, with your doctor.

  • Monitor Symptoms: Keep track of your menstrual cycle and any unusual symptoms to report to your healthcare provider.

Don'ts

  • Remove IUD Without Consultation: Always consult your doctor before removing your IUD.

  • Skip Regular Check-Ups: Continue regular gynaecological check-ups to ensure your IUD is in place and functioning correctly.

Ignoring the need for proper fertility testing can lead to delays in identifying underlying issues that could affect your ability to conceive.

An IUD does not interfere with most fertility tests, so there is no reason to avoid testing if you are considering pregnancy in the future.

How to avoid complications with an IUD

  • Communicate openly: Maintain open communication with your healthcare provider about your fertility goals and the presence of your IUD.

  • Follow medical advice: Adhere to your doctor's recommendations regarding the timing of IUD removal and fertility testing.

  • Stay informed: Educate yourself about how your IUD works and how it interacts with your reproductive system.
